Mandatory Parent or Guardian Consent Form for Google Additional Services

It is required that one parent or guardian complete the form attached to this post to indicate whether or not you give approval for your child to use additional Google services in school.

This form must be completed for each child and will need to be completed annually. Click forms above this post to complete the form.

If you do not complete the form by November 1st, your child will not be able to use the additional Google services. 

Please note, a Google Form was previously sent out in an email via the Sapphire portal. We are asking that you please complete the form on ParentSquare even if you already completed the Google form that was in the email.
