The Evolution of our Partnership!


St. Luke's University Health Network

In May 2015, St. Luke’s Miners Campus was awarded a three-year Rural Health Outreach grant from the Health Resource and Service Administration.  This grant was submitted with a Memorandum of Understanding Partnership Agreement between St. Luke’s Miners, Panther Valley School District and community partners.  Through this outreach, the School District becomes the hub of the community where there will be daily opportunities and outreach to connect students, families and community members to care and to work together to improve health outcomes.

The collaborative, rural health outreach opportunities align with St. Luke’s Community Health where community partners and the school district (including students and families) work together in promoting healthy living initiatives, chronic disease prevention, connecting students and families to medical, dental, mental health, substance abuse prevention and programs, and leadership opportunities for any interested students and families.

In using the School District as a hub, students, staff and community members and partners are actively involved in assessing, implementing and evaluating initiatives and outreach through focus groups, committees and student leadership opportunities throughout the year.

Since 2015, this partnership has flourished, providing significant growth to the community health umbrella of initiatives and services provided to the Panther Valley Community!

Examples of the outreach have included:

  • Full and Part Time School Based Coordinators available to assist/engage any students/families in connecting to care and/or growing in leadership opportunities.
  • St. Luke’s Mobile Youth Health Center.
  • Vision screening and assessment.
  • Star Community Dental Services
  • Healthy Relationship Education for 7th - 8th grade plus district wide messages and resources.
  • Nutrition Education and Garden Programs for all 3rd, 4th and 5th plus district wide messages and resources
  • Community outreach education through Panther Valley events to promote St. Luke's services, educate community members, and volunteer within the community. 
  • Get Your Tail on the Trail and Live Your Life Programs throughout the District/Community.
  • Community Supported Agriculture from Leiby’s Farm included in local Summer Lunch Programs in Summit Hill, Coaldale, and Lansford
  • Comprehensive sports medicine services
  • Sports performance training
  • Cross-trained sports medicine and community health staff
  • On-site family development specialist 
  • On-site food pantry at the Elementary School
  • Hygiene and necessities pantry and resources
  • Career Linking Academy 
  • Annual health carnival
